Patient Rights & Responsibilities

  1. Written information on rights and responsibilities of patients and family shall be given.
  2. Staff shall also provide information on the same.
  3. Translators or pictorial depictions shall be used to enhance communication.
  1.  Privacy during clinical interviews, examination with a chaperone in case of patient or opposite gender, procedures/treatment and transport from one location to another within the hospital and also during transfer out.
  2. The patient/the guardian or surrogate will case the patient is not able to exercise the right, shall decide who (in case the patient is not able to exercise the right), shall decide who will receive and what information is to be given regarding their care and under what circumstances.
  3. In case of any statutory requirements the patient information shall be released to the appropriate authority.

  2. Proposed treatment with risks, benefits, side effects and possible alternatives
  3. Expected outcomes
  4. Unexpected events during surgery or medication or other treatment like physiotherapy etc
  5. Problems related to recovery
  6. Possible results of not receiving treatment
  1. Summary of patient’s condition, test results, current diagnosis, and recommendations for treatment shall be given to the patient if the opinion is sought from a doctor from the Forti-in-house doctors, the patient records are accessible.
