Patient Rights & Responsibilities

  • Patient and family shall be informed about their rights and responsibilities in a manner and language that is understandable.
    1. 1. Written information on rights and responsibilities of patients and family shall be given.
    2. 2. Staff shall also provide information on the same.
    3. 3. Translators or pictorial depictions shall be used to enhance communication.
  • The patient has a right to privacy and confidentiality of care and information.
    1. 1. Privacy during clinical interviews, examination with a chaperone in case of patient or opposite gender, procedures/treatment and transport from one location to another within the hospital and also during transfer out.
    2. 2. The patient/the guardian or surrogate will case the patient is not able to exercise the right, shall decide who (in case the patient is not able to exercise the right), shall decide who will receive and what information is to be given regarding their care and under what circumstances.
    3. 3. In case of any statutory requirements the patient information shall be released to the appropriate authority.
  • Right to know the name of treating doctor, care plan, progress & information on their health
  • Right to access all medical records regarding their care which would be provided to patient and family.
  • Right to determine their information and that of his/her guardian or surrogate, in case the patient is not able to exercise the right.
  • Right to receive information about all aspects of medical care and treatment from the treating doctor regarding:
    1. 1. Diagnosis
    2. 2. Proposed treatment with risks, benefits, side effects and possible alternatives
    3. 3. Expected outcomes
    4. 4. Unexpected events during surgery or medication or other treatment like physiotherapy etc
    5. 5. Problems related to recovery
    6. 6. Possible results of not receiving treatment
  • Any change of plan of care or condition/ prognosis shall be discussed with the patient and/or family and documented.
  • Information on the expected cost of treatment.
  • Respect for personal dignity, privacy and treatment, including chaperone when a patient whose gender is different from the medical staff, is interviewed, examined, treated and transported.
  • Refuse being photographed, video graphed or have any of their treatment records published or used for any research or publicity.
  • Protection from any form of abuse or neglect.
  • Treating patient information as confidential.
  • The hospital shall facilitate the patient's right to seek a second opinion without fear of compromise to his/her care within or outside the hospital.
    1. 1. Summary of patient's condition, test results, current diagnosis, and recommendations for treatment shall be given to the patient if the opinion is sought from a doctor from the Forti-in-house doctors, the patient records are accessible.
  • When informed consent will be required and the process for the same
  • Refusal of treatment or diagnostic procedures.
  • Any special needs, lingual, cultural or spiritual, without compromising other patient's and care.
  • Information and consent before any procedure, anesthesia, blood transfusion, extension of scope of the treatment, risks involved and estimated cost.
  • Information on how to voice a complaint, conflicts and differences of opinion about patient care.

Action taken shall include

  • Investigation by hospital management.
  • Resolution of the same with patients and families being involved in the process.
  • Measures to prevent recurrence if applicable.
  • Information and consent before any research protocol is initiated.
  • Patient valuables shall be handled as per procedure on valuable handover and patient shall be informed regarding this.

Patient Responsibilities

  • Providing accurate information about their health, including past illnesses or problems, hospitalizations, allergies and current or past use of medication.
  • Reading all forms, including consents and understanding them, and seeking clarifications if needed, before signing them.
  • Following the treatment plan recommended by the health care provider.
  • Adhering to the facility policies, including those regarding smoking, use of mobile phones, home food, noise and visitors.
  • Respect hospital personnel and infrastructure in accordance with the tenets of Indian law, cultural norms and human rights.
  • Responsible to ensure no valuables are on the person of the patient.
